Partner with Us
Our Adult Day Health program provides clinically supported care, meaningful activities, and compassionate supervision—helping older adults thrive while easing caregiver burden.
Your proactive referral will allow LIFE Senior Services to follow up and contact the family directly, increasing the likelihood that the services you recommend will be put in place.
LIFE's Family Assistance Coordinators offer personalized service in the following ways:
Sets up meetings with you, your patient and/or your patient's family to discuss the many ways that Adult Day Health provides daytime support.
Contacts and follows up with your patient and their family caregiver to encourage follow-through on your recommendations.
Answers questions or arranges for a tour to get the enrollment process started.
