PACE FAQ
How can I tell if I'm eligible for the LIFE PACE program? To join you must:
- Live in the LIFE PACE defined service area
- Meet the nursing facility level of care requirements as determined by the Oklahoma Health Care Authority
- Live in the community without jeopardizing your health or safety
- Be age 55 or older
What is nursing facility level of care?
Generally, individuals needing assistance with two or more personal care needs or home care needs such as laundry, housekeeping and meal preparation.
Are prescription drugs covered?
Yes. All prescription and non-prescription drugs deemed necessary by the LIFE PACE interdisciplinary care team are covered by LIFE PACE.
Are people who do not qualify for Medicaid eligible for LIFE PACE?
Yes. There are multiple payment options depending on the individual circumstances. These specific situations can be discussed in detail with a LIFE PACE enrollment specialist.
How do participants get to LIFE's Adult Day Health Center?
LIFE PACE provides transportation to the Adult Day Health Center/LIFE PACE center and the new LIFE PACE Alternative Care site near 31st and Sheridan . Transportation is a key part of the PACE benefit. Transportation is not only provided between the home and the day health center(s), but also to appointments with specialists and other activities as approved by the LIFE PACE interdisciplinary team.
Do LIFE PACE participants attend LIFE's Adult Day Health Center every day?
Participants can attend daily if necessary. Adult Day Health attendance is based on individual needs.
I'm interested in joining, what do I do next?
First, our enrollment specialist will speak with you over the phone for basic information, questions and concerns. If you wish to pursue enrollment, a home care assessment is scheduled with the LIFE PACE intake coordinator and a registered nurse. Next, we offer a tour of LIFE'S Adult Day Health/LIFE PACE Center and the LIFE PACE Alternative Care center (if requested). Once enrolled, the interdisciplinary team will complete assessments.
What services does LIFE PACE provide to those who participate?
Delivering all needed medical and supportive services, LIFE PACE provides the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible. Services may include the following:
- Adult Day Health that offers nursing; physical, occupational and recreational therapies; meals; nutritional counseling; social work and personal care;
- Medical care provided by a LIFE PACE physician familiar with the history, needs and preferences of each participant;
- Home health care and personal care;
- All necessary prescription drugs;
- Social services;
- Medical specialties, such as audiology, dentistry, optometry, podiatry and speech therapy;
- Respite care;
- Hospital and nursing home care when necessary
Can I disenroll?
Yes - if a participant moves out of the service area or simply wishes to end their enrollment, they must notify us. Disenrollment occurs at the end of the month so the client can be re-enrolled in Medicare and Medicaid programs. In addition, LIFE PACE will arrange alternative community services for the participant. While we will do all we can to support a participant the program relies on the participant and family to work together to ensure the safety of the participant in the community.
