LIFE PACE provides transportation to the Adult Day Health Center/LIFE PACE center and the new LIFE PACE Alternative Care site near 31st and Sheridan . Transportation is a key part of the PACE benefit. Transportation is not only provided between the home and the day health center(s), but also to appointments with specialists and other activities as approved by the LIFE PACE interdisciplinary team.
Participants can attend daily if necessary. Adult Day Health attendance is based on individual needs.
First, our enrollment specialist will speak with you over the phone for basic information, questions and concerns. If you wish to pursue enrollment, a home care assessment is scheduled with the LIFE PACE intake coordinator and a registered nurse. Next, we offer a tour of LIFE'S Adult Day Health/LIFE PACE Center and the LIFE PACE Alternative Care center (if requested). Once enrolled, the interdisciplinary team will complete assessments.
Delivering all needed medical and supportive services, LIFE PACE provides the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible. Services may include the following:
Yes - if a participant moves out of the service area or simply wishes to end their enrollment, they must notify us. Disenrollment occurs at the end of the month so the client can be re-enrolled in Medicare and Medicaid programs. In addition, LIFE PACE will arrange alternative community services for the participant. While we will do all we can to support a participant the program relies on the participant and family to work together to ensure the safety of the participant in the community.
