Individual Care Plans

The plan of care serves as a road map; it is a written agreement developed by the participant and the interdisciplinary healthcare team that defines how and when services will be provided to the client. It is formulated by the team after all of the initial assessments are completed. The team then integrates goals that the family and client would like to achieve. The care plan is reviewed every six months or if a significant change occurs. Care plans are key in communicating expectations for both the organization and the client.

The interdisciplinary team is led by Dr. Robert Gray, an Oklahoma native who graduated from Oral Roberts University and completed his medical residency at OU-Tulsa in 1981. Most recently practicing at St. John in Owasso, Dr. Gray specializes in geriatrics and adult medicine. 

LIFE PACE services may include the following:

  • Primary and specialty care visits
  • Transportation
  • Adult Day Health
  • Physical, occupational and recreational therapies
  • Mental and behavioral health
  • Nutritious meals and nutritional counseling
  • Personal care (bathing, dressing, grooming)
  • Light housekeeping and laundry
  • Home health care
  • Prescription drugs and medication management
  • Social services
  • Medical specialties, such as audiology, dentistry, optometry, podiatry and speech therapy
  • Durable medical equipment
  • Respite care
