Diabetes Empowerment Education Program (DEEP) and Chronic Kidney Disease Wordkshop

Six-part interactive series is designed for people with diabetes, pre-diabetes and those caring for someone with the disease. Topics include risk factors, nutrition, physical activity, use of a glucose monitor, working with a care team, accessing community services and complications, including chronic kidney disease (CKD). Snacks provided.

Dates: Wednesday, June 13, 20, 27 & July 11, 18, 25

If you are newly eligible or soon to be eligible for Medicare, this seminar is for you. Seminar will cover the basics of Medicare and what you need to know to make an informed decision about your coverage options.

This seminar is intended for Medicare beneficiaries and closed to licensed Health Insurance Agents and anyone affiliated with a Health Maintenance organization or other conflicts of interest.
