LIFE’s Legacy of Family Caregiving Award Nomination

LIFE’s Legacy of Family Caregiving Award celebrates an individual who has cared for a senior-aged loved one with compassion and commitment to dignity and support as well as continued dedication to their own well-being. The award recipient may be an adult child, a spouse or another family member of the care recipient. Nominees should demonstrate their merit for this award through the following means: (a) the ability to admit when they need help; (b) the ability to seek out and/or accept assistance from others, including public/community support; (c) a commitment to continued personal education about aging and/or disease specific to their care recipient; (d) a continued commitment to personal well-being and engagement in their own life; and (e) the maintained ability to see and embrace the humor in the facts of life.

Using the space below or by uploading a file, please explain why you think the individual you are nominating deserves the award in no more than 500 words.

The Selection Process:
Nominees will be reviewed and voted upon by a selection committee made up of aging services professionals. The selection committee will review nominations after the submission deadline on March 16, 2018. All decisions will be finalized by March 20, 2018.

Recognizing Award Recipients:
Award recipients in each category will be publicly recognized and presented with a Legacy Award at an hors d’oeuvres and spirits reception.
