LIFE’s Legacy of Volunteerism Award Nomination

LIFE’s Legacy of Volunteerism Award celebrates an individual senior (55+) who has made an impact in their local community by devoting their time and talents through volunteerism and/or mentorship. This may be accomplished through any of the following means: (a) independent volunteerism; (b) volunteerism through an organization; (c) a formal advisory position; (d) a formal mentoring program; (e) an informal relationship with a mentee or group of mentees.

Nomination essays should discuss the nominee's most prominent award-deserving qualities and the actions.

Using the space below or by uploading a file, please explain why you think the individual you are nominating deserves the award in no more than 500 words. Please be specific.

Additionally, nominations may be faxed or mailed to Erin Shackelford at LIFE. All mailed entries must be received by March 15, 2019. Call (918) 664-9000, ext. 1215 for more information.

The Selection Process:
Nominees will be reviewed and voted upon by a selection committee made up of aging services professionals. The selection committee will review nominations after the submission deadline on March 15, 2019. All decisions will be finalized by March 20, 2019.

Recognizing Award Recipients:
Award recipients in each category will be publicly recognized and presented with a Legacy Award at an hors d’oeuvres and spirits reception.
